Indiana Unemployment Application

When you get ready to fill out the IN unemployment application you need to make sure you have set aside plenty of time to complete the application, and you need to be sure that you have all of the information ready that may be requested of you.

You need to know:

1. Your social security number.
2. Your driver's license number.
3. Your last employer's name and address.
4. The dates that you worked at your last employer.
5. The dates of all of the jobs that you have held in the past (This is not always required, but is very helpful to have).

If you are filling out a Indiana unemployment application in person, you will need to take the following items with you:

1. Your social security card.
2. Your driver's license.
3. Last paystub from your previous employer.
4. Any layoff documents your previous employer may have given you.

I had to quit my job in West Virginia as a result of my husband accepting a job in Indiana. Do I qualify for unemployment benefits in Indiana?

In 2012 I transferred from MN to work at the Chicago Assembly Plant in Chicago, IL. Until recently, I've been living in a hotel in IN - we just officially moved into an Indiana home two weeks ago from MN. ( I've been paying MN state taxes and IL taxes). Which state do I apply for unemployment in? I've also completed my waiting week from MN (July 2012) and would like to know if I would need to have another waiting week for a different state. The Plant will be down next week which is the reason for my unemployment. It will resume production in the beginning of January 2013.

Hi, 2 weeks ago, I had a heart attack on the job,(driving),d ue to extreme blockages. I had 2 stents put in place in 1 artery. I still have 60% & 40% blockage in 2 more arteries. I had been employed with the same company for the past 9 years driving a truck for their deliveries. Load up, unloading, clean up were all of my responsibility & the job was stress filled. At 54 years old with zero health ins. provided by them, I let them know 2 days ago that I cannot drive for them any longer. I am now unemployed. Can I draw unemployment ? I live in Indiana & was employed in Indiana. Thank you for your response.
